Fill the form and bring it to the ATI when you come for the selection test.

Suppose if you answer YES to any of the questions in Section 1, please contact me immediately (before the exam date) and get guidance. (Our email address is atitrincomalee@sliate.ac.lk). If you have any official document showing that you are safe now, please send a scanned copy of the document to me as a reply to this email account.

Read the Health and Safety Guidance (Annexure 01) carefully, and act accordingly.

Don’t forget to bring a duly filled health declaration form, admission card, identity card, and face mask on the exam date. Without these, you will not be allowed to sit the exam.
